We participate in Medicare and a variety of HMO and PPO insurance plans. Please contact our office if you have any questions or concerns regarding specific plans. Many insurance plans require pre-authorization from your primary care physician. It is the patient’s responsibility to obtain such authorization prior to the appointment visit. Failure to obtain an authorization may result in rescheduling your appointment or denial of claims.

Please bring a copy of your insurance card at the time of your appointment. We file all claims with Medicare and all primary and secondary insurances.

In the event we are not contracted with a specific plan or you are a self-pay patient, financial arrangements can be made with our office. Please come prepared at your appointment to be responsible for your co-pay, deductibles or any other remaining balances. We accept cash, checks and all major credit cards.
